During onsite inspection, Licensor observed a pull cord window blind capable of forming a loop, at the licensed front door entrance of the childcare. (2)(b) During onsite inspection, Licensor observed straps on the diaper changing pad capable of forming a loop. (2)(c) During onsite inspection, Licensor observed plastic wrap around the diaper changing pad, and the infant crib mattress, posing a suffocation hazard. (2)(d) During onsite inspection, Licensor observed the sliding glass door leading to the outdoor play area was not equipped with placement stickers or art work at children’s eye level. (2)(f) During onsite inspection, Licensor observed boards with nails and splintered edges near the emergency exit gate, posing a risk of cuts, abrasions, punctures. (3)(a) During onsite inspection, Licensor observed shelving equipment and household bikes in poor conditions accessible in the outdoor play area. (3)(g)
